Provide for all foods to be procured from an approved wholesale processing establishment and provide for all foods to come from a source that is in compliance with all laws relating to food and food labeling. Observed the following food products to be improperly stored: 1) uncovered in chest freezers. Provide for all food products in storage to have proper covers. Food covered. Immediate/Onsite
15  Observed the following improper non-food contact surfaces: 1) broken outer molding on 3-door cooler at front kitchen, 2) rust on inside shelving of 3-door cooler at front kitchen, 3) rust inside most of the chest freezers along the walls right by butchers area, 4) rust on wire racks in back, 5) rust on shelving in walk in cooler. Provide for all food contact surfaces to be smooth, easily cleanable, non-absorbant, corrosive resistant, and light colored. (750.690) Next Inspection
23  Observed the following contact surfaces to be unclean: 1) outside, front of microwave oven in front kitchen, 2) inside of all chest freezers along walls in butchers area. Clean. (750.800) Next Inspection
29  Observed the following plumbing to be improperly installed or maintained: 1) corroded base of faucet on front kitchen hand sink. Replace faucet; provide for all plumbing to be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code. 32  Observed no paper towels in employee rest room. Provide for all handsinks to have supply of soap and paper towels to assure that proper handwashing occurs. (&50.1130) Next Inspection
36  Observed the following floor areas to be in poor repair: 1) covebase in butchers area not properly adhered to wall, 2) no seal on concrete flooring in butchers area and back, 3) no seal on concrete floor in walk in cooler, 4) no floor to wall juncture in employee rest room (covebase). Provide for all flooring areas to be smooth, easily cleanable, non-absorbent, durable surfaces, and be maintained in good repair. (750.1200) Next Inspection
37  Observed the following wall/ceilings area(s) to be in poor repair: 1) chipped wall surface in front kitchen on back side of counter, 2) gap between wall and 3-bay sink in butchers area, 3) chipped paint on wall behind chest freezers (east wall), 4) holes / chipped paint on wall around back door. Provide for all walls and ceilings to be smooth, easily cleanable, non-absorbent, durable, and light colored and be maintained in good repair.
